- Clients on a Chronic Disease Management (CDM) Medicare care plan. Note: There is a game fee for CDM clients as we are a private practice.
No Doctor’s Referral Needed
Unless a Worker’s Compensation claim, Department of Veteran’s Affairs (DVA), CTP 3rd Party, Chronic Disease Management (CDM) plan.
Consultation
All consultation rooms are completely private for client comfort and peace of mind. Clients can be assured of dignity and privacy. The length of the sessions will vary depending on the complexity of the condition and injury.
Bring Along
- Appropriate and comfortable clothing if possible – shorts and tops. We do offer gowns and shorts.
- Any Doctor’s letters, referrals, medical imagines (x-rays, MRIs, CT scans, Ultra sound scans, bones scans, etc).
- Any details regarding insurances if you have a claim (workers’ compensation, 3rd party).
What to Expect
At Optimum Health & Performance we will listen to you and take the time we need to understand your full story. We may also ask you to fill out some questionnaires to gain a further understanding on your presenting injury or pain. Following a thorough examination, we will discuss the findings with you and give you a diagnosis. From there we will draw up a rehabilitation and management plan specific and unique to you that will get you back to doing what you love.
Cancellation Policy
If we have your mobile number on our system, a reminder SMS will be sent out the day prior to your appointment. If you are late to your appointment you may only have a shorter consult, in order not to impact the following patients. This will be at our discretion.
It is a policy that we require a minimum of 24-hour notice for any cancellation. This will allow us to contact other patients on our waiting list. We will attempt to best accommodate you and reschedule your appointment. Late cancellation if unreasonable and non-attendance will attract a cancellation fee.
